We present the Caregiver of the Year award to someone who goes “above and beyond” to provide care and support to a current or former dialysis patient. Renal professionals, as well as friends and loved ones of patients, are eligible for this award.
To nominate someone for the American Kidney Fund’s Caregiver of the Year Award, simply complete the form below. The person who is selected as the Caregiver of the Year will be honored at The Hope Affair on Wednesday evening, October 17, 2012, at Mellon Auditorium in Washington, D.C.
