The Learn it! Check it! Live it! Toolkit

The Learn it! Check it! Live it! Toolkit can help adults at risk for kidney disease, diabetes or high blood pressure to understand their risk factors, the screening tests  and the importance of a healthy lifestyle in preventing kidney disease.  The materials in the toolkit can be used in either a group setting or for one-on-one counseling.

Welcome Book

The Welcome Book provides detailed instructions to help you prepare for and conduct a successful educational session.

Flipchart

The Flipchart provides the content and slides for an educational session.  It covers kidney disease, including its risk factors, important screening tests, and steps to a healthy lifestyle to promote kidney health.
