How to Apply

Please note: We have reached program enrollment capacity due to the extremely high demand for this program. No additional applications will be accepted for the 2009 enrollment year.  As funding allows, patients who are currently waitlisted will be enrolled in the program in the order that requests were received.  Applications for the 2010 enrollment period will be accepted beginning November 1, 2009.

Either the dialysis patient or the patient's renal social worker may submit an application for the Medicare Part D Grant Program for Prescription Bone Medications.

After AKF reviews the application, AKF will mail any correspondence regarding application status or requests for additional information to the patient’s dialysis center.

Patients who are accepted into the program will receive an AKF pharmacy benefit card in the mail at their home address, unless they request otherwise during the application process.

Completed applications may be faxed to (301) 881-3311 or mailed to American Kidney Fund, 6110 Executive Blvd., Suite 1010, Rockville, MD 20852.
