How to Apply

Dialysis center social workers may submit Safety Net Grant applications for their patients to the American Kidney Fund.

The American Kidney Fund does not accept applications directly from patients.

If you are a patient who is in need of a Safety Net Grant to help with treatment-related expenses not covered by insurance, speak with your social worker.

If you are a social worker who wishes to submit a grant application for a patient, visit Online Patient Services or call our Patient Services Department at 1-800-638-8299.
