The American Kidney Fund greatly appreciates what you do as a coach. Logging your educational activities and sessions helps us keep track of coaches' progress. Please do not forget this very important step. Give us as much information as you can. Your honest feedback will help us improve the program. Click the link to SurveyMonkey to complete your log form. We encourage coaches to submit their event and activity logs promptly after each session is conducted to ensure accurate and timely reporting.
